new pains in a flare Options

Hi - hoping you can help me work out what is what! My rheumy has increased the methotrexate I've been taking since Feb to 8 tablets a week and pred up to 20mg a day. When I was seen at the end of April myDAS was 6.8 and ESR 60. I've since felt dreadful and last weeks blood test my ESR was 107, so the nurse is going to contact the rheumy this week.

Thus far my pains have been mainly elbows to fingers and toes to knees. Now I have a strange pain kind of round my rib cage - sort of where my bra ends. Is this arthritis too, or something else? My right shoulder and neck are sore too.

I have been signed off by the school OH nurse till at after half term initially, but feel really bad about it as GCSE exams are iminent and I have top set Y11. Sorry for the waffling, appreciate hearing from you XX Ailsa

Im sorry you are feeling so bad at the moment.I get pains in my ribs quite alot and i was told it was the arthritis.Some nights i can't wait to get my bra off, not a pretty sight!
I have also got it in both my shoulders and neck.
Maybe you need your drugs changing.
It's not your fault that you cannot work at the moment RA always seems to flare up at the wrong time.

sorry that you are so unwell, esr up there- you cant be feeling goodSad

the rib thing is to do with inflammatory process. i have something called costocondritis but there is also something called pleuritic pain that can cause the same.

i find rest is the only way to get through- if you can have steroid then ask for a depomedrome injection. its a big lump of steroid in one go. you must rest after it though for full effect. the gp can do this or you can have it at the rheumatology unit.

Please don't feel bad about being off work! I understand where you are coming from, the worry, guilt, about letting down your Year 11 students; however at this point in time surely they are on revision material anyway and top set students should be able to get their acts together with a supply teacher for guidance. At the end of the day your health is paramount and if you don't get it right now who knows what the future holds ... RA is such an unpredictable disease!

I agree totally with Poppy and Jenni, the chest pains will likely be inflammatory and most likely Costochondritis; inflammation of the cartilage of the ribs, in particular, the cartilage that joins the ribs to the breastbone, at the front of the chest. It is a separate condition but often occurs alongside RA. Neck and shoulders will probably be the RA.

It certainly sounds as though you are going through a flare and a review of the meds would seem necessary. If you don't hear anything today I would follow up with a phone call to rheumatology because you could do with something more to get things under control quickly. Sorry you are feeling so rough and hope you get some relief very soon. BUT please don't be tempted to dash back to work too soon!!

Firstly, as Jenni says, with an ESR up there you must be feeling pretty rough.

I think you have to put yourself first - your y11's wll be on revision now and if they're not, it's a bit late anyway!
